I have a Word document that I would like to convert to a PDF that can ask for information from a user. It needs to have the appropriate text boxes to enter data, properly formatted phone number cells (U.S. phone numbers), calendar drop-down boxes for areas you need to enter dates, signature boxes for signatures, Initial boxes for Initials, etc.
The successful candidate must provide a full document and allow me 2 rounds of review with any additional revisions that are required to meet the expectation.
I will provide guidance and input as necessary throughout the contract.
