Intake and Eligibility Specialist is responsible for the day-to-day management of application review and award activities. The Intake and Eligibility Specialist has basic knowledge of the principles, concepts, and methodology of reviewing applications, determining eligibility, and demonstrated skills in applying this knowledge in carrying out assignments, operations, or procedures. Responsibilities Review of applications for completeness and accuracy. Ensure applications are complete for program review (form completed correctly and all required documents are accurate). Contact program applicants and request any/all missing or incomplete information. Update applications with correct or missing data. Make initial eligibility determinations based on individual program requirements. Maintain accurate, UpToDate case notes in applicant files. Provide excellent customer service to program participants and accurately respond to inquiries about the program.

Eligibility Specialist
Eligibility Specialist provide case management services to an assigned caseload of low- and moderate-income applicants seeking rental and utility assistance in the wake of the COVID-19 Pandemic. • Processes client applications for assistance and serves as the client’s primary representative for the applicant. • Review intake forms to determine client’s eligibility for program benefits, make recommendations for award, generate agreements for execution, and, if required, perform income verification to fulfill funding requirements. • Serves as primary point of contact; expected to provide technical assistance to clients to assist with the completion of required program forms and other documents. • Responsible for ensuring any services provided to the client are documented and organizing documents in program tracking systems. • A production-based program, so clearing assigned caseloads through queues is an essential job function.

Assess child and family needs to create referrals and linkages to services such as subsidy and medical coverage. Facilitate removal and placement of children into safe and stable care while also supporting the families and care givers during the process. Develop and maintain quality case and program written documentation that reflects client progress and also meets CHS, funders, and DCF requirements. Travel to conduct home visits to access children’s living environments and provide recommendations to their families and caregivers in a supportive manner. Assist foster families in caring for the foster children in their home. Attend dentist and doctors appointments as well as pick up and drop off to and from school with children as needed. Support and care for foster parents and children through the dependency process. Build relationships and work closely with therapy centers, counselors, rehab centers, schools, teachers, etc. to better support families and children.
