Clinical Safety Manager & Serious Incident Investigator. Scope of the role is to lead on the coordination and management of system based patient safety incident investigations using Root Cause Analysis [RCA] and a variety of investigation processes to review the period covered for the purpose of fact-finding and critical data analysis. To identify the relevant information in order to identify areas of improvements, good practice, contributing factors and the lessons learnt to improve the quality and safety of services. To present the chronology of events to Care Review Panel and lead on the investigations. Compile the finding of the final reports for Scrutiny Panel and Coroner's Inquest.
